Job Description

The Department of Hospital Internal Medicine is seeking a qualified Nurse Practitioner or Physician Assistant to join our Advanced Care at Home (Hospital at Home) division.

This position offers a unique opportunity to provide a mix of in-person and virtual patient care to hospitalized patients through our virtual platform 24/7. This program is vital to Mayo Clinic's strategic growth plan and the future of care -- Bold. Forward. Unbound.

The Advanced Practice Provider works with our team of Physicians, Nurses, Case Management, Pharmacy and Administration to provide quality and strategic care to patients in the comfort of their home using community service providers.

Our providers are expected to stay up to date with the latest evidence-based medicine and provide comprehensive care for a medically complex and diverse patient population.

NP/PAs take part in all aspects of the care of the hospitalized ACH patients from acquisition (admission/transfer), daily rounding, coordination of care with consultants and specialists, patient education, and discharge. Our NP/PAs take part in 24/7 care including nights, holidays, and weekends on a rotation structure.

As a novel program, Advanced Care at Home providers, including NP/PAs, have vast opportunities to support Mayo Clinic's 3 Shields -- clinical practice, education, and research. Within the department there are many opportunities to partner with colleagues in research, quality improvement and education allowing for continued professional growth within your career.

Ideal candidates for the Advanced Care at Home NP/PA position include an extensive medical knowledge base with the desire for continued learning and growth as a health care provider, adaptability with a changing medical landscape and problem-solving skills to approach challenges with a mindset to find creative and effective solutions.
