Key Responsibilities

The Physical Therapist will evaluate, plan, and provide personalized physical therapy treatment to patients in their homes to help them achieve maximum functional independence. They will also collaborate with an interdisciplinary team and maintain accurate clinical documentation in accordance with regulatory standards.

Requirements

Candidates must be a graduate of an accredited Physical Therapy program and hold a valid Physical Therapist license. Additional requirements include a driver's license, automotive insurance, and CPR/BLS certification.
