Join Advocate Health as a Physical Therapist – Home Health, $10K Sign-On Bonus in Hazel Crest, Illinois, United States. Department: 39122 South Metro D - Home Care Physical Therapy Status: Full time Benefits Eligible: Yes Hours Per Week: 40 Schedule Details/Additional Information: Weekend rotation currently scheduled for once every 5 weeks (Sat/Sun). One summer and one winter holiday per year.
