Join Country Manor as a Physical Therapist - Full-Time in Sartell, Minnesota. Full Time Physical Therapist- $5,000.00 Sign On Bonus Champion Rehab is looking for a compassionate, innovative and energetic Physical Therapist to join their award-winning therapy team!

Key Responsibilities

• Direct client care: continuous assessment of patient needs and functional deficits, patient care/treatment, on-going modifications to existing plan of care as needed, ability to initiate appropriate referrals to other disciplines when needed.

• Willingness to work closely with skilled and dedicated interdisciplinary team in order to maximize patient satisfaction and overall success based on diagnosis and comorbidities.

• Applying clinical expertise as necessary including but not limited to family and staff education/training, recommendations and guidance for safe and successful discharge, supervision of Physical Therapy Assistant, communicating with Physicians to ensure fluency of care.

• Documentation which follows regulatory guidelines (state, federal).

Requirements

• Graduate of an accredited Physical Therapy program.

• Currently licensed to practice in State of MN. New graduates and temporary license holders encouraged to apply.
