About the Role

Join Country Manor as a Physical Therapist (PT) - Home Health - Sign-on Bonus in United States. Physical Therapist - Home Care $5,000 Sign-on Bonus Monday-Friday, NO nights, weekends, or holidays! Paid mileage reimbursement.

Key Responsibilities

  • Assess, develop, implement, and revise client's individualized plan of care for ordered Physical Therapy services.
  • Observe, record, and report the client's reaction to treatment and any changes in the client's condition to the physician, and RN Case Manager.

Requirements

  • Must have active Licensure for Physical Therapy in the state of MN.
  • Must have Driver's License, auto insurance and reliable vehicle.
  • Willing to travel to patient's home to perform evaluations/visits.

Benefits & Compensation

  • Salary: $42-$58.00/hr

Work Schedule

  • Full-time position.
  • No weekends required.
  • Day shift only - no nights.
