About the Role

Join Country Manor as a Physical Therapist (PT) - Home Health - Sign-on Bonus in United States. Physical Therapist - Home Care $5,000 Sign-on Bonus Monday-Friday, NO nights, weekends, or holidays! Paid mileage reimbursement.

Key Responsibilities

Assess, develop, implement, and revise client's individualized plan of care for ordered Physical Therapy services.

Observe, record, and report the client's reaction to treatment and any changes in the client's condition to the physician, and RN Case Manager.

Requirements

Must have active Licensure for Physical Therapy in the state of MN.

Must have Driver's License, auto insurance and reliable vehicle.

Willing to travel to patient's home to perform evaluations/visits.

Benefits & Compensation

Salary: $42-$58.00/hr

Work Schedule

Full-time position.

No weekends required.

Day shift only - no nights.
