Nurse Practitioner- Hospitalist (Part-Time & Casual)

St. Croix Health is seeking a highly skilled and motivated Nurse Practioner to join our healthcare team in our Medical/Surgical department.

Join a highly collaborative and patient-centered Hospital Medicine team where you will have the opportunity to practice at the top of your license while making a meaningful impact in a rural community. As an APP Hospitalist, you will work alongside experienced physicians, nurses, and interdisciplinary team members to provide comprehensive care for adult inpatients, including admissions, inpatient management, and discharge planning.

Responsibilities

Patient Assessments: Conduct comprehensive history and physical examinations, perform daily rounds, and complete clinical evaluations.

Diagnosis & Management: Manage acute and chronic medical conditions, initiate appropriate diagnostic and therapeutic interventions, and order/interpret lab work and imaging.

Collaborative Care: Work closely with multidisciplinary teams—including physicians, nurses, case managers, pharmacists, and social workers—to coordinate optimal inpatient treatment.

Transitions & Discharge: Facilitate safe transitions of care, coordinate post-discharge services, and educate patients and their families regarding ongoing recovery plans.

Documentation: Maintain accurate, timely medical record documentation in the electronic health record (EHR).

Requirements

Education and Licensure

• Master's degree in Nursing and completion of a Nurse Practitioner program
• Active Wisconsin: Nursing license and certification as a Nurse Practitioner
• Minimum of one year NP experience
