The physical therapist will evaluate patients in their homes to develop and implement customized treatment plans to restore function and independence. They are also responsible for providing accurate documentation and communicating effectively with patients, families, and the interdisciplinary team.
Candidates must be graduates of an accredited Physical Therapy program and hold or be eligible for a New York state license. The ability to travel to patients' homes within the assigned geographic territory is required.
