You will partner with primary care providers to deliver evidence-based behavioral health interventions and whole-patient care. You will also leverage real-time data to track patient outcomes and contribute to a collaborative clinical team.
Candidates must hold a Master's degree in a behavioral health field and possess an active, good-standing license such as LAC, LPC, LMSW, LCSW, AMFT, or LMFT. A strong commitment to patient impact and the ability to work collaboratively in an integrated care setting are required.
