Key Responsibilities

The Behavioral Health Manager conducts biopsychosocial assessments and provides evidence-based therapeutic interventions for dialysis patients within an integrated care team. They also collaborate with nephrologists and social workers to develop, document, and adjust individualized care plans to improve patient health outcomes.

Requirements

Candidates must hold an active independent license in a behavioral health field and possess a Master's degree or higher. A minimum of two years of clinical experience with patients facing chronic medical conditions and co-occurring mental health challenges is required, along with bilingual proficiency in English and Spanish.
