The Pharmacist will provide clinical care and comprehensive medication management for a home-based patient population in collaboration with an interdisciplinary team. They are responsible for developing treatment plans, educating patients and providers, and ensuring adherence to evidence-based medication strategies.
Candidates must hold a Doctor of Pharmacy (PharmD) degree and possess 3 to 5 years of experience in an ambulatory care setting. A current, unrestricted pharmacist license is required, along with proficiency in Microsoft Office Suite and clinical documentation systems.
