Key Responsibilities
The Nurse Practitioner will provide comprehensive care to senior Medicare patients, including creating care plans and performing health assessments. They will work collaboratively with an integrated care team to support complex patients in dialysis centers and home settings.
Requirements
Candidates must hold a current Oregon Nurse Practitioner license and be board certified by the ANCC or AANP. A minimum of 2 years of clinical experience in primary care, family medicine, internal medicine, gerontology, or palliative care is required.
