Key Responsibilities

The Nurse Practitioner will provide comprehensive, one-on-one care to senior Medicare patients, including conducting health assessments and creating dedicated care plans. They will work collaboratively with an integrated care team to manage complex health needs and ensure high-quality, cost-effective outcomes.

Requirements

Candidates must hold a current Maryland Nurse Practitioner license and possess at least 2 years of clinical experience in primary care, family medicine, internal medicine, gerontology, or palliative care. A valid DEA license, CPR/BLS certification, and knowledge of HCC coding are also required.
