Key Responsibilities

The Nurse Practitioner will provide comprehensive care to senior Medicare patients, including conducting health assessments and managing complex care plans. They will work collaboratively with an integrated care team to coordinate services and improve clinical outcomes for dialysis and primary care patients.

Requirements

Candidates must hold a current Washington Nurse Practitioner license and be board certified by the ANCC or AANP. A minimum of 2 years of clinical experience in primary care, internal medicine, gerontology, or palliative care is required.
