This role involves the consistent application of knowledge to deliver appropriate, safe, efficient, and cost-effective pharmaceutical care, including reviewing medication orders for interactions and optimizing therapy based on evidence-based interventions. The pharmacist is responsible for patient care, ensuring accurate medication dispensing, overseeing preparation and delivery, and providing patient counseling and education.
Candidates must possess a minimum Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution and hold a current license to practice pharmacy in the state of practice, including dispensing controlled substances. Preferred qualifications include a PGY2 Critical Care Residency and two or more years of Critical Care clinical pharmacist experience.
