Centra’s PACE (Program of All-Inclusive Care for the Elderly) is a nationally
recognized model of care serving adults aged 55 and older in our community. The
mission of PACE is to deliver person-centered care in the setting that’s most
appropriate for each participant—whether that’s in their own home, with family,
in long-term care, or assisted living.
PACE is a capitated model of care, which means there are no productivity
metrics, only a focus on delivering the care each participant truly needs.
Everything is coordinated with a dedicated interdisciplinary team of providers,
nurses, dietitians, physical and occupational therapists, social workers,
activities coordinators, and transport staff.

Centra Medical Group (CMG) is a comprehensive network of nearly 600 employed
providers within Centra Health. Centra Health services a population of 600,000
community members with 70 different outpatient and specialty practices. Centra
offers primary care and specialty services across Central and Southern
Virginia—from Lynchburg to Danville, Farmville, Moneta, and Bedford. CMG
includes family practices, specialists, and surgeons in fields such as
cardiology, neurosurgery, psychiatry, urology, and more.
Key Responsibilities
The physician will join the PACE (Program of All-Inclusive Care for the Elderly) team to deliver person-centered, comprehensive care to adults aged 55 and older in a community-based setting. This role involves collaborating with a dedicated interdisciplinary team to manage participant needs within a capitated model focused on quality care rather than productivity metrics.
Requirements
Candidates must hold an MD or DO degree and have required experience working with older adults. Board Certification or Eligibility is required in Family Medicine, Palliative/Hospice/Internal Medicine, or Emergency Medicine, with preference for experience in palliative, hospice, primary care, or long-term care settings.
