Entity: Owensboro Health Regional Hospital | Department: Case Management | Work Hours: 8a-4:30p | Weekend Requirements: N/A | 1.0 FTE
Job Summary
Facilitates cost-effective, high quality patient transitions of care and outcomes by collaborating with staff and providing guidance, education, and support to assigned patients.
Job Responsibilities
Coordinates and manages discharge planning activities.
Completes a comprehensive assessment of the patient's overall medical condition, financial status, emotional status, and support system to develop a discharge plan for post hospital care.
Collaborates with patients, families, and healthcare team members, including physicians and nurses.
Provides patients and families with information and education on post hospital care options and available resources.
- Facilitates patient referrals to external healthcare facilities and/or other community services
- Ensures all necessary paperwork and documentation are completed accurately and timely
- Monitors patient progress and modifies the discharge plans as necessary
Communicates with Manager, Director, and/or Physician Advisor on complex discharge planning issues and delays in service.
Trains new employees, students, or interns as needed.

- Work Experience
- A minimum of 1 year relevant experience preferred
Education
- Bachelor's degree or higher in Social Work required upon hire
- Licensure/Certification/Registration
- LSW - Licensed Social Worker in the State of Kentucky required upon hire OR
- LCSW - Licensed Clinical Social Worker in the State of Kentucky required upon hire OR
- CSW - Certified Social Worker in the State of Kentucky required upon hire
