Key Responsibilities
The Nurse Practitioner will provide advanced palliative care to patients in their homes, collaborating with the Palliative Medical Director to develop and implement individualized care plans. They are responsible for performing patient assessments, managing treatment plans, and facilitating referrals while maintaining accurate clinical documentation.
Requirements
Candidates must hold a Master of Science in Nursing and have at least two years of experience as a Nurse Practitioner. A current RN license, Nurse Practitioner certification, and a valid driver's license are required, with ACHPN certification preferred.
