PRN-Medical Social Worker (MSW) - Home Health

Key Responsibilities

The Medical Social Worker assists with client admission by evaluating psychosocial status and developing/implementing the social work component of the interdisciplinary care plan. Responsibilities also include observing and reporting changes in social factors, consulting with physicians, maintaining clinical records, and participating in discharge planning.

Requirements

Candidates must possess a Master's degree from an approved school of social work and must have or be acquiring certification from the Academy of Certified Social Workers. A minimum of two years of experience is required, with at least one year in a healthcare setting, along with strong verbal and written communication skills.
