Medical Social Worker - Home Health PRN

Key Responsibilities

The Medical Social Worker assists in client admission by evaluating psychosocial status and developing/implementing the interdisciplinary care plan related to social work. Responsibilities also include observing and reporting social changes affecting the client's illness and consulting with the physician regarding treatment plan alterations.

Requirements

Candidates must possess a Master's degree from an approved school of social work and be in the process of acquiring certification from the Academy of Certified Social Workers. A minimum of two years of experience is required, with at least one year in a healthcare setting, along with strong communication skills and the physical ability to access client homes.
