Key Responsibilities

The Hospice Social Worker assesses the psychosocial status of patients and families related to terminal illness, communicating findings to the interdisciplinary group and providing necessary counseling or crisis intervention. Responsibilities also include educating on cultural needs, maintaining clinical records, assisting with advanced directives, and participating in the individualized plan of care development.

Requirements

Candidates must possess a Master’s degree in social work, although those with a Bachelor's degree in Social Work are eligible to apply with pay adjusted accordingly. A minimum of two years of documented supervised experience in healthcare, preferably hospice, is required, along with an understanding of hospice philosophy and death/dying issues.
