Join HarmonyCares as a Geriatric Primary Care NP/PA – No Weekends, No Call in Okemos, MI, US. Geriatric APP role available with growing primary care team! Monday-Friday, no weekends/evenings, managing chronic conditions of a high-risk patient panel in a value-based team setting.

Key Responsibilities
Provide primary care services to high-acuity patients within their homes as part of a value-based team. Manage chronic conditions for a high-risk patient panel while ensuring personalized, quality-based healthcare delivery.
Requirements
Must be a licensed Nurse Practitioner or Physician Assistant with a valid DEA license and CPR/BLS certification. Experience in home health, geriatrics, and primary care is required.
