Key Responsibilities
The Case Manager / Certified Wound Care Nurse will coordinate resident care from admission to discharge while leading the facility's Skin Integrity and Wound Care Program. They will collaborate with interdisciplinary teams to develop evidence-based treatment plans and mentor nursing staff on best practices.
Requirements
Candidates must hold a current Washington State RN or LPN license and possess an Associate Degree in Nursing. A minimum of two years of nursing experience in skilled nursing or rehabilitation is preferred, along with CWCN certification.
