Key Responsibilities

Provides skilled physical therapy treatments in a home health setting according to the physician's plan of care. Collaborates with the healthcare team to assess patient functional levels, set goals, and educate patients and caregivers on injury management.

Requirements

Requires an active Michigan Physical Therapy license, a valid driver's license, and reliable transportation. Previous home health care experience is preferred.
