This position offers weekends off. Work with Crescent Hospice in Mount Pleasant, South Carolina, United States.
The RN Case Manager will deliver hands-on hospice nursing care and coordinate services for patients within the assigned tri-county territory. They will also collaborate with the Clinical Director to assess patient eligibility and provide ongoing support and education to patients and their families.
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in computer programs and EMR systems, along with a valid driver's license and reliable transportation, is required.
