Key Responsibilities
Collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs and discharge planning. Conduct psychosocial assessments and coordinate with community agencies to ensure appropriate post-discharge care.
Licensure & Certifications
Requires a Master's Degree in Social Work and a State of Michigan license. Experience in family assessment, crisis intervention, and grief counseling is preferred.
