Key Responsibilities
The Social Worker is responsible for assessing patient needs and providing direct social work services and interventions throughout the admission and discharge process. They will also provide ongoing case management support and collaborate with staff, patients, and families to develop effective care plans.
Requirements
Candidates must hold a Master of Social Work (MSW) degree from an accredited school and possess a valid Connecticut State Social Work Licensure (LCSW). Experience with utilization management, community resources, and knowledge of healthcare regulations is highly beneficial.
