Key Responsibilities
Develop and implement individualized, patient-centered care plans for patients with complex medical conditions. Collaborate with an interdisciplinary team to coordinate care, optimize patient outcomes, and facilitate safe discharge planning.
Requirements
Requires an Illinois Occupational Therapy license and BLS certification. A bachelor's degree is mandatory, with a master's or doctorate preferred, along with prior inpatient experience.
