The Occupational Therapist will evaluate and treat patients in their homes to improve their daily living and work activities. They are responsible for designing individualized treatment plans, documenting progress, and coordinating care with the broader rehabilitation team.
Candidates must be a licensed Occupational Therapist in Minnesota with a valid driver's license and reliable transportation. At least one year of experience and proficiency with OASIS and WellSky documentation systems are strongly preferred.
