Key Responsibilities

The physical therapist will interpret physician referrals to conduct comprehensive patient evaluations and implement individualized treatment plans. They are responsible for monitoring patient progress, adjusting care plans, and completing all necessary clinical documentation and discharge planning.

Requirements

Candidates must hold a Bachelor's degree in Physical Therapy and a valid state license in Wisconsin or Illinois. Excellent communication skills and the ability to perform physical tasks such as lifting and patient transfers are required.
