Social Worker

Key Responsibilities

The Social Worker will assess the psychosocial status of patients and families related to terminal illness, providing counseling, crisis intervention, and education on practical needs and advanced directives. Responsibilities also include participating in the individualized plan of care, collaborating with the interdisciplinary team, and assisting with end-of-life planning matters.

Requirements

Applicants must possess a master’s degree in social work or an undergraduate B.A. Degree in Social Work, along with a minimum of two years of supervised experience in healthcare, preferably hospice. Candidates need to understand the hospice philosophy, be flexible with work hours, and demonstrate a willingness to maintain knowledge of applicable software programs.
