Key Responsibilities
Provide compassionate, patient-centered palliative care to individuals with serious illnesses within skilled nursing facilities. Collaborate with interdisciplinary teams to develop treatment plans, manage symptoms, and facilitate goals-of-care discussions.
Requirements
Requires an active Nurse Practitioner license in Massachusetts and at least two years of post-certification experience. Experience in palliative care, SNF, hospitalist, emergency department, or ICU settings is preferred.
