Key Responsibilities
The physician will provide advanced primary and specialty care to geriatric and medically complex patients in their homes or care settings. They are also responsible for providing medical leadership, mentoring clinicians, and coordinating care with interdisciplinary teams to improve patient outcomes.
Requirements
Candidates must hold a Doctor of Medicine or Osteopathy degree with a current unrestricted state license and board certification in family practice, internal medicine, geriatric, or palliative care. A minimum of 2 years of experience in healthcare, specifically in acute, geriatric, or long-term care, is preferred along with prior supervisory experience.
