Key Responsibilities
The social worker assesses the psychosocial needs of patients and families to develop and implement effective care plans and complex discharge strategies. They collaborate with the multidisciplinary team to ensure efficient care and provide expertise in social systems and crisis intervention.
Requirements
Candidates must hold a Master's degree in Social Science or Social Work and possess a valid Social Worker license. At least one year of experience in clinical healthcare or community social work is required.
