## Responsibilities
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans
- Deliver skilled PT services to home health patients across a variety of diagnoses and conditions
- Monitor patient progress and adjust care plans accordingly
- Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated, patient-centered care
- Maintain accurate, timely clinical documentation in compliance with state and federal regulations
- Educate patients and family members on home exercise programs and safe mobility techniques

## Qualifications
Active Texas Physical Therapist (PT) License
— required
- Home health experience preferred; new graduates with clinical experience are welcome to apply
- Strong communication, time management, and organizational skills
- Reliable transportation and valid driver's license
- CPR certification preferred
