Key Responsibilities
The social worker performs psychosocial, emotional, and spiritual assessments to develop and implement goal-directed care plans for hospice patients. They also provide crisis intervention, family counseling, and serve as an active care advocate within an interdisciplinary team.
Requirements
Candidates must hold a Master of Social Work (MSW) degree from a CSWE-accredited school. One year of home health care experience and hospice experience are preferred, along with a valid driver's license and reliable transportation.
