Key Responsibilities
The Nurse Practitioner will provide advanced palliative care to patients in their homes, collaborating with the medical director to develop and implement individualized care plans. Responsibilities include performing comprehensive assessments, managing treatment plans, and facilitating effective communication among the care team and families.
Requirements
Candidates must hold a Master of Science in Nursing and have at least two years of experience as a Nurse Practitioner. A current RN license, Nurse Practitioner certification, and a valid driver's license are required, with ACHPN certification preferred.
