Key Responsibilities

Provide comprehensive Care Management Social Work services across various adult and pediatric sites within the Legacy Health System. Conduct psychosocial evaluations, transition planning, and crisis management while collaborating with interdisciplinary teams and community stakeholders.

Requirements

Requires a Master of Social Work degree and at least 2 years of relevant experience in care management, mental health, or medical social work. Must hold or obtain LCSW and LICSW licensure and maintain BLS Provider certification.
