Key Responsibilities

The social worker evaluates the psychosocial and mental health needs of patients to facilitate transition planning, crisis management, and service coordination. They provide holistic, trauma-informed care while advocating for patients and their families within an interdisciplinary team.

Requirements

Candidates must hold a Master of Social Work degree and possess either a CSWA or LCSW license in Oregon. Relevant experience in mental health, addictions, or medical social work is required for this role.
