Key Responsibilities

The Clinical Social Worker evaluates patients' psychosocial and mental health needs to facilitate transition planning, crisis intervention, and service coordination. They implement intervention strategies for behavioral health, addiction, and end-of-life care while advocating for patients and their families within an interdisciplinary team.

Requirements

Candidates must hold a Master of Social Work degree and possess the appropriate clinical licensure (CSWA or LCSW) in Oregon or Washington. Experience in care management, mental health, or medical social work is required, along with BLS Provider certification.
