System Hemostatic and Antithrombotic Stewardship Pharmacist

Key Responsibilities

This role involves monitoring and evaluating drug therapy for appropriateness based on patient status, institutional guidelines, and cost efficiency, while assuming responsibility for managing pharmacy services in assigned areas to improve patient outcomes. Responsibilities also include providing drug information, participating in patient care teams, and precepting students and residents.

Requirements

Required education is a Bachelor of Science in Pharmacy, or a PGY1 Pharmacy Practice Residency (or 5 years of related specialty experience in lieu of residency), with a Doctor of Pharmacy and PGY2 Specialty Residency preferred. Candidates must be licensed by the LA Board of Pharmacy and possess strong computer skills, effective communication abilities, and leadership/teaching skills.
