Speech Therapist Pathologist

Location: Adult Day Health Care, Lancaster, Ca

Shift: Part Time

Position Purpose: The Speech-Language Pathologist, under the supervision of the Program Director, is responsible for the delivery of skilled, high-quality direct patient care to participants of JWCH, Wesley Health Centers. This role includes conducting speech-language assessments, developing individualised plans of care, providing appropriate therapeutic interventions, monitoring participant's progress, and completing required clinical documentation in accordance with applicable policies, procedures, and regulatory requirements.

Principle Responsabilities

1. Conduct an initial assessment of each participant prior to enrollment and ensure all assessments are dated and signed.

2. Develop an individualized plan of care for speech-language therapy or an appropriate maintenance program based on the participant’s assessment and identified needs.

3. Conduct reassessments when a participant has an existing plan of care or when referred by the multidisciplinary team (MDT), and update the plan of care as appropriate.

4. Complete documentation following each treatment session, using narrative notes or flow sheets as appropriate, including the treatment provided and the participant’s response. Documentation must be dated and signed by the individual providing the service and countersigned by the Speech-Language Pathologist, as applicable.

5. Complete quarterly progress notes documenting the participant’s status, progress, and ongoing treatment needs in accordance with applicable requirements, including PRL B-2.

6. Review and complete the participant’s discharge summary, ensuring the documentation is dated and signed.

7. Provide therapeutic activities, services, and procedures appropriate to the participant’s needs and consistent with the assessment findings and individualized plan of care.

8. Provide interventions intended to reduce or minimize functional limitations and support the participant’s recovery following illness or injury.

9. Support and promote the participant’s physical, emotional, and social well-being through appropriate therapeutic interventions.

10. Assist participants in developing and maximizing functional abilities to the highest level appropriate to their individual needs.

11. Provide guidance and instruction in the use of therapeutic, creative, and self-care activities designed to improve or maintain functional abilities.

12. Collaborate with the multidisciplinary team and other healthcare professionals to support coordinated participant care.

14. Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for participants, staff, and visitors.

15. Participate in required safety and compliance trainings and apply learned practices in daily work activities.

16. Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace.

17. Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards.

18. Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role.

19. Participate in JWCH’s customer service training called AIDET on an annual basis.

• - Demonstrate an understanding of AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) to apply in participant encounters or team discussions.
• - Utilize the AIDET communication framework to communicate with participants, caregivers, and team members in a manner that reduces anxiety, improves understanding, and supports positive clinical outcomes.

20. Perform other duties as assigned.

Requirements

1. Graduate from a school of Speech-Language Pathology or an accredited speech-language pathology program.

2. Hold a current, valid California license to practice as a Speech-Language Pathologist.

3. A high degree of technical skill, integrity, initiative, and interpersonal skills.

4. Knowledge of and special interest in working with older adults and adults with special needs.

5. Knowledge of applicable California regulatory requirements related to speech-language services in community-based adult health settings.

6. Strong assessment, treatment planning, clinical documentation, and communication skills.

7. Ability to work collaboratively with an interdisciplinary healthcare team.

8. Experience providing speech-language services in a community-based, adult day health, rehabilitation, or similar healthcare setting is preferred.
