The PACE Per Diem Physician provides direct participant care, including examinations, diagnoses, treatment, and care planning, while championing the PACE model of care. This role involves active collaboration with the Interdisciplinary Team (IDT) and coordination of care across various settings, including after-hours on-call duties.
Candidates must possess an MD or DO with an unrestricted Oregon medical license and required CPR certification, DEA certificate, and valid driver's license. A minimum of one year of experience working with the elderly or frail population is mandatory, with preferred experience including five years as an attending physician and background in value-based healthcare or PACE settings.
