Key Responsibilities

The RN Triage serves as the primary liaison between patients, families, and neurologists to assess needs and coordinate comprehensive memory care services. Responsibilities include managing prior authorizations, maintaining accurate patient records, and implementing nursing care plans in accordance with organizational policies.

Requirements

Candidates must possess a current, valid RN license and BLS certification, with at least one year of experience in an acute care or physician practice setting. Strong clinical judgment and excellent communication skills are required to support patients navigating Alzheimer's disease and related dementias.
