Our flagship workforce development program, Ready, Willing & Able (RWA), operates across 660 transitional housing beds and combines housing and comprehensive supportive services with paid work training, career preparation, and occupational skills training. The Doe Fund’s innovative social enterprise model creates additional opportunities for New Yorkers to gain paid, hands-on experience while delivering essential services across the city, including supplemental sanitation, graffiti removal, and rain garden maintenance. We are also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 2,100 apartments in operation or development, creating safe, dignified homes with supportive services and spaces that foster community and connection. The Doe Fund has an annual operating budget of $88 million and a full-time staff of over 500, a significant portion of whom are graduates of our programs.

We are seeking a dedicated Case Manager/Mental Health Counselor for the Villa House Permanent Supportive Housing Program, which provides long‑term residential care and 24‑hour support services for homeless men and women living with HIV/AIDS. The program is designed to maximize residents’ independence while ensuring their physical and emotional security within a supportive community environment.

Forty-one units in the residence are occupied by individuals referred through the New York City HIV/AIDS Services Administration (HASA), and the remaining twenty-seven are apartments for adults referred by the New York City Housing Authority (NYCHA).

The Case Manager/Mental Health Counselor is responsible for delivering supportive case management services to HASA‑funded residents. The ideal candidate will demonstrate strong clinical skills, excellent communication abilities, and a deep understanding of the unique needs of individuals living with HIV/AIDS.

Key Responsibilities

* Maintain a caseload and provide regular case management services to these individuals

* Maintain resident records in compliance with agency and funding standards

* Conduct and review assessments

* Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency

* Monitor and ensure residents are linked to healthcare providers

* Provide counseling and crisis intervention as necessary

* Assist residents in securing and maintaining benefits

* Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs

* Facilitate groups and activities that promote health and independent living skills development

* Monitor and assist residents with rent payments and budgeting

* Conduct regular unit inspections

* Educate residents about approaches to mental health

* Compile resources on mental health information and treatment strategies

* Attend ongoing training to enhance the services provided to the residents

* Assist with completion of HASA’s weekly and monthly reports as well as any other relevant reports

* Other duties as assigned

The Ideal Candidate

* You have a Master’s Degree in Social Work or related field; a license in Clinical Social Work or Mental Health Counseling is preferred.

* You have at least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illness

* You have at least 5 years of experience in the mental health field

* You have knowledge of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issues

* You have solid computer skills – internet, Microsoft Office, and charting database

* You have great organizational, writing, and communication skills
