Key Responsibilities
The Telehealth Nurse Practitioner will conduct comprehensive virtual health assessments and document risk adjustment findings for Medicare and Medicaid members. They are responsible for closing quality care gaps and providing clear care plans to support value-based care outcomes.
Requirements
Candidates must hold active Nurse Practitioner licenses in at least two of the specified states and possess at least one year of experience in Family or Internal Medicine. Familiarity with HEDIS, HCC risk adjustment, and the ability to commit to a minimum of 24 hours per week are required.
