Key Responsibilities
The Social Worker collaborates with multidisciplinary teams to perform psychosocial assessments, develop discharge plans, and provide crisis intervention for patients. They are responsible for coordinating care, managing referrals, and documenting patient information in accordance with hospital policies.
Requirements
Candidates must hold a Master's degree in a Social Services field and possess an Indiana LSW or LCSW license. A minimum of 2 years of experience in acute care, mental health, or community healthcare is preferred.
