Key Responsibilities
The social worker will provide person-centered case management, counseling, and advocacy for older adults as part of an interdisciplinary team. Responsibilities include conducting home visits, maintaining accurate clinical documentation, and assisting families with Medicaid processes.
Requirements
Candidates must hold a Master's degree in Social Work and possess a current Michigan social work license. At least two years of experience in a health-related field working with the elderly or frail population is desired.
