Director of Rehabilitation

Lead a clinical program that is part of the community—not apart from it.

Northern California | Folsom & Elk Grove

We are seeking a Director of Rehabilitation to lead and grow a comprehensive rehabilitation program within a premier Tenfold Senior Living community in Northern California.

  • You will build a strong team, establish clinical standards, partner with community leadership and create a rehabilitation program that residents, families and operators recognize as an essential part of the community.

What You'll Lead

As Director of Rehabilitation, you will be responsible for the day-to-day clinical and operational leadership of the rehabilitation program.

Clinical Leadership

  • Provide direct physical, occupational and/or speech therapy as appropriate to your discipline.
  • Establish a culture of strong clinical reasoning, evidence-based practice and resident-centered care.
  • Review documentation and clinical performance.
  • Support clinicians with complex cases and clinical decision-making.
  • Ensure evaluations, plans of care, progress reports and discharge planning reflect the resident's actual goals and needs.
  • Promote appropriate utilization and high-quality outcomes.

Team Leadership

  • Recruit, onboard and develop rehabilitation clinicians.
  • Create accountability around productivity, documentation, scheduling and clinical quality.
  • Mentor therapists and assistants.
  • Establish a collaborative environment where clinicians can grow.
  • Lead regular clinical and operational meetings.

Community Partnership

  • Develop strong relationships with executive directors, wellness leadership, nursing and resident services.
  • Participate in community meetings and identify residents who may benefit from therapy.
  • Build appropriate referral pathways.
  • Partner with the community's wellness and fitness programming.
  • Help position rehabilitation as part of the resident's broader lifestyle and independence—not simply a service delivered after a decline.

Program Growth

  • Monitor census, referrals, evaluations, visits and clinical utilization.
  • Identify opportunities to expand appropriate services.
  • Help develop new service lines as community needs evolve.
  • Use Prava's reporting and technology to understand what is happening inside the practice and where intervention is needed.

This Isn't a Traditional Rehab Department

  • We aren't looking for someone who simply wants to manage a schedule.

You might work with a resident after a fall—but your responsibility doesn't end with getting them through a course of treatment.
