Job Description
Now Hiring - Psychiatric-Mental Health Nurse Practitioner
Salary Range $125,000 to $140,000 a year
The Advanced Practice Nurse will be responsible for, but are not limited to:
1. Conduct visits to patients in accordance with local, state and federal
regulations.
2. Communicate visit findings to designated members of the interdisciplinary
team.
3. Facilitate access to appropriate support models of care (i. e.,
rehabilitation, on-going aggressive treatment).
4. Consult with other members of interdisciplinary team to contribute to the
patient's plan of care.
5. Prepare and maintain accurate patient records, charts and documents.
6. Assist in development of clinical practice guidelines/standards in support
of quality care as needed.
7. Ensure collaboration with patients, caregivers, medical directors and
members of IDT.
Education
Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an
accredited program.
Professional Experience
* Must have a Nurse Practitioner's license in the state of operations
* PMHNP certification
* Must have 1+ years experience working within Family, General Practice,
Geriatric care or related field
* Experience in a nursing home, hospital, assisted living or acute rehab space
is a plus but not required.
* Commitment to ongoing professional development and clinical excellence.
* Must have a valid driver's license and a reliable source of transportation
* Must be willing to travel locally
