Join Bluestone Physician Services as a Nurse Practitioner/Physician Assistant – Primary Care, Flexible Schedule in United States.
Key Responsibilities
- manage patients’ chronic conditions, address social determinants of health, manage transitions to and from.
- provide behavioral health support and more.
- Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities F.
- Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.
